MEdical MArijuana Card Evaluation
To receive a medical marijuana card in the state of Missouri, residents must be approved by the Missouri Department of Health and Senior Services. Residents are required to submit a certificate signed by a physician that qualifies the patient as a candidate for the Department of Health & Senior Services's review and approval. Below are the steps you can expect to follow to certifying with us in order to apply for your Missouri medical marijuana card with the Missouri Department of Health and Senior Services.

What To Expect
We recommend that you register first as a patient with the State of Missouri at the Missouri Medical Marijuana Portal (also known as the MMMP Registry). The Registry is where you will eventually submit your complete application. Once your account is setup there, please:
- Complete our registration form by clicking the "Book Appointment" button below. You will be required to review and agree to our terms and conditions in order to submit your appointment request.
- Make your payment of $250 for the history and physical evaluation fee online (this is required before scheduling your appointment).
- A 24/7 Healthcare team member will contact you within 1-2 business days to schedule and confirm your appointment.
- The appointment itself will last 10 to 30 minutes. Please be prepared to bring your medical history records. You may also upload supporting documentation via the online registration form.
- If approved for certification at your appointment, you will receive a "Physician Certification Form" (PCF) signed by your evaluating physician. You may download, print, and bring in your own partially completed PCF. Please hand it to your evaluating physician at the start of your evaluation.
- Submit your complete application within 30 days of receiving your signed PCF the Missouri Department of Health and Senior Services.
- Once your application is received, it may take approximately 14 days to approve and issue your Missouri Medical Marijuana card.
